PRINCIPLES OF THE TREATMENT AND GIVING SERVICES TO PATIENTS

Please, read the regulations valid in Prima-Dent surgeries, which are based on the many-year experience and are meant to facilitate cooperation. Thank you for visiting our surgery and for the trust you have given to our staff.

1. Treatments have a 1-year written guarantee. The patient obtains a treatment card, which is the guarantee of the performed treatment. Losing the card equals losing the guarantee (a xerocopy, if was made, will be accepted), The card includes: a general medical interview a diagram (a schematic description of the original condition of the teeth), the planned treatment (at your request also expected cost of the treatment), date of visit, description of the performed treatments, medical diagnosis, kinds of materials, used, the price.

Correct and complete filling in of the documentation is doctor's duty.
The patient should regularly read entries to the card and, in case of doubts, ask the doctor for an explanation.

2. (At present we are introducing a computer system. Patient's condition history will be present in the computer. We ask our patients to read the entry to the computer after each visit. At patient's request we will print the history of the condition or give information from a given visit on an electronic data carrier. Giving this information to patients will be free of charge).

3. All doctors working in Prima-Dent surgeries are warrants of the performed treatment (e.g. if during the warranty period the patient will lose the filling, every doctor working in Prima-Dent is obliged to perform a new filling free of charge because of the warranty).

4. Conditional treatments are not covered by warranty (the doctor explains the patient in detail why he cannot give warranty for a given treatment and writes in the card information about a conditional treatment). Conditional treatments are made with utmost care (as all other treatments) in accordance with the current technical level and medical knowledge.

5. Before performing the operation the doctor is obliged to inform the patient about visit costs (present various cost variants of the operation). The costs may be changed during the operation. Then the doctor, if it is possible, should stop the operation and inform the patient about the change in treatment conditions.

6. The pricelist of Prima-Dent is in the secretary of every surgery and can be read there by Patients. Operations which are highly specialist, demanding extraordinarily large quantities of material can differ from prices given in the pricelist, nevertheless, the cost of such an operation has to be agreed upon with the patient beforehand.

7. If for any reason the patient wishes to continue the treatment at another Prima-Dent doctor's, it is enough to express this wish in the secretary - without the need to explain the decision.

8. Patients with pain are served as first.

9. Every patient is written for a specified day, specified hour and all our staff make their best to keep to the scheduled time of the visit. In case of extended waiting, we ask our Patients for understanding. The specific character of our work prevents us from specifying exact time of the operation.

10. If on the day of operation the Patient does not have enough money on them, they can (at the consent of the doctor performing the operation) a short-term credit. The patient has to have a document certifying their identity.

11. In case of payments higher than PLN 500, discounts will be taken into account only when paid in cash.

12.If the Patient has doubts about the quality of the performed operation, they can at any time demand consultation with another Prima-Dent doctor or with the Head of surgeries, Dr Przemysław Uliasz.
